We provide care for both insured and uninsured patients. BCHS accepts medicare, Medicaid and most private insurances. Those who are uninsured may qualify for a sliding fee scale.
How do I make an appointment?
To schedule an appointment, you will need to call BCHS at 504.533.4999. Appointments are given on a first called, first served basis, and fill quickly each day. If all slots are filled for
the day at the time you call, you can be placed on a wait-list in case an appointment becomes available.
How much does it cost?
If you do not have insurance, the cost of your visit is based on a sliding scale of your income. Primary care visits will cost between $25 - $100. Behavioral health visits will cost
between $20 - $50. While we are not a free clinic, we do believe that everyone should have access to health care.
Can I get an appointment after normal work hours?
Appointments are only available during regular clinic hours: Monday-Friday, 8:30am –5:00pm.
Is BCHS a free clinic?
BCHS is not a free clinic. If you do not have insurance, the cost of your care is based on a sliding scale of your income.
What do I bring with me to my appointment?
If you are a new patient, please bring: 1) your photo ID, 2) proof of insurance, 3) any prescription medications. If you do not have insurance, you will need to bring 1) proof of your income
and 2) $30-$60 to pay for your visit.
As proof of income you may provide one of the following: Prior year tax return, recent pay stubs, pensions, social security benefits, disability, veteran’s benefits, unemployment
compensation, retirement, child support or alimony payments, letter with balance on Electronic Benefit Transfer (EBT) card, or reference letter. All of these proofs of income must be from the
past 30 days or less.
If income is paid in cash, please provide a letter from employer to include the current date, date hired, employee’s name and address, employer’s name/address/phone number, wages
per hour and how many hours of work per week, and frequency of pay period. The letter must be signed and dated by the employee and employer.
Can I fill out the forms before I arrive for my appointment?
Yes, if possible, we encourage you to fill out the forms before you arrive at the clinic. The forms can be accessed here. The forms are also available at the clinic, so you can fill them out
at your appointment if you prefer.

Is my visit confidential?
Yes, your visit is completely confidential. Your information is protected by law (HIPAA) and will only be disclosed as allowed by law or with your authorization.
Do you provide contraception? What types?
Our providers are able to discuss with you and provide prescriptions for oral contraception. On-site contraceptives are available.
