Baptist Community Health Services (BCHS) is proud to announce that it has officially achieved Patient-Centered Medical Home (PCMH) certification, a nationally recognized milestone that affirms BCHS’ commitment to delivering high-quality, patient-centered care.

The Patient-Centered Medical Home is a model of care designed to place patients at the center of their healthcare experience. PCMHs focus on building strong, ongoing relationships between patients and their clinical care teams while improving access to care, coordination across services, quality outcomes, and patient engagement.

Research shows that PCMH-certified practices improve patient health outcomes and the overall patient experience, increase staff satisfaction, and reduce healthcare costs through more effective and efficient care delivery. Practices that earn PCMH recognition demonstrate a sustained commitment to continuous quality improvement and a patient-centered approach to care.

“Achieving PCMH recognition sets a high bar for our patient care—one that we have aspired to reach and are excited to reach,” said Dr. Hannah Pounds, Chief Medical Officer of BCHS. “It is a true testament to teamwork, collaboration, and our shared commitment to excellence in patient care.”
